Taking Out a 401k hardship withdrawl

I have a 401k of $ 5943.18, and because I have been on short term disability since 10/6/2010, there are a lot of financial difficulties accumulating.

I don't foresee the possability of me returning to work , since I will not be able to fullfill my job requirements anymore after the wrist surgery.
So I was wondering if you could possibly tell me all the taxes and penalty amounts, or if I would qualify for the waiver, and maybe direct me to a webpage of the letter I need to write to the 401k company.

I’m sorry to hear that. If you take money out now you’ll probably have to pay a 10% penalty plus taxes on the money that you withdraw.

I would contact your old company’s human resources department to get more information and to see if you need to fill out anything to take a hardship withdraw or if you can just send them a letter with proof of your hardship. And where to even send it too
